FHCA PRESENTATION/ ENDORSEMENT REQUEST

 

Fort Hill Civic Association requests that those who wish to present information at a monthly meeting or who request from the organization any kind of endorsement (Letters-of-Support, endorsement of zoning waivers, building permits, etc.) complete the form below and submit it to an organization representative PRIOR to any meeting with the Association. If a request for endorsement is being submitted, this form must be submitted giving us enough time to distribute/make it available to the membership prior to the meeting you wish to attend. 1.     Provide name of individual and/or organization making the request, including name, address, email, telephone information.

 

2.     Nature of request

·        What is being requested of the FHCA? Please explain in detail.

·        What kind of response is need from the FHCA? When do you need a response?

·        To whom should correspondence be addressed (please provide name, title, organization name, address, phone number)?

 

3.   Project Information (if applicable)

·        What is the name of the project?

·        Where is this project located?

·        How will this project affect current residents (good/bad)?

·        How will this project benefit the Highland Park community? 

 

4.   Zoning Variances (if applicable)

If you are requesting support for a zoning variance, please attach a copy of the completed application and provide the information requested below:

1.   Names of abutters?  Have abutters been contacted? (we strongly urge that you make certain all abutters and other neighbors in the immediate vicinity receive a flyer about this discussion regarding your request prior to the meeting of the FHCA.)

2.   If this request concerns the creation of housing in a new or existing building, how many bedrooms will there be and how many people (max.) will it be designed for?

3.   How will parking be addressed?

4.   Has a hearing been scheduled?

5.   Next steps.

 

N.B. Please be aware that the chairs may not be able to put you on the next month's agenda, if there are already too many items.   When you do come to present your project, you will need to answer any questions that people may have.  After this you will be asked to leave the meeting.  The association will discuss the project and then make a decision which will be emailed to you within two weeks.
