Terms and Conditions For Ordering Information from Equifax Credit Information Services, Inc. via the Internet

1. You can only order your own personal Equifax Credit Profile and, by clicking the "Agree" button, you certify that you are the person whose Equifax Credit Profile is being requested.
   
2. You must be 18 years or older to order your Equifax Credit Profile and, by clicking the "Agree" button, you certify that you are 18 years or older.
   
3. It is illegal to obtain information on a consumer (other than yourself) from Equifax Credit Information Services, Inc.. If you do so, you may be fined up to $5000.00 or imprisoned up to one year in jail.
   
4. Online ordering of your personal Equifax Credit Profile will require the use of a secure browser such as Netscape Navigator 2.0 or higher or Microsoft Internet Explorer 3.0.
   
5. Equifax Credit Information Services, Inc. recognizes the privacy of the information that you will be providing on the order form. This program encrypts the data on the order form before transmission to Equifax, and the data is decrypted only upon receipt by Equifax.
   
6. The price per copy is $8.00 in most states. Several states have unique regulations or require different fees. Refer to the chart below for your state of residence:

If you have been denied credit or some other benefit within the last 60 days and Equifax was used in the decision process, you are entitled to a free copy of your credit profile. If this applies to you, please call 1-800-997-2493. (7 days a week)

MANY GOVERNMENT RECORDS ARE AVAILABLE FREE OR AT A NOMINAL COST FROM GOVERNMENT AGENCIES. CREDIT REPORTING AGENCIES ARE REQUIRED BY LAW TO GIVE YOU A COPY OF YOUR CREDIT RECORD UPON REQUEST, AT NO CHARGE OR FOR A NOMINAL FEE.
