BENEFITS

The following benefits are provided when loss results from injuries sustained through accident.  All claims for benefits are are subject to the provisions, conditions and limitations of the Constitution and By-Laws of the Association.
1. Death while commercial airline fare-paying passenger on scheduled timetable flight:

$50,000.00

2. Death while on commercial chartered airline flight as fare-paying customer:

2,000.00

3. Death while acting as pilot, co-pilot, flight engineer or attendant on flight of incorporated licensed licensed passenger airline carrier, but not on charter flight: 5,000.00
4. Death while fare-paying passenger in incorporated licensed land conveyance: 50,000.00
5. Death from other accidental causes: 5,000.00
6. Loss of Both Eyes: 5,000.00
7. Loss of Both Hands or Both Feet by severance above wrist or ankle: 5,000.00
8. Loss of One Hand and One Foot by severance above wrist and ankle: 5,000.00
9. Loss of One Hand or One Foot by severance above wrist or ankle: 2,500.00
10. Loss of One Eye: 2,500.00
11. Loss of Four Fingers from either hand by severance: 1,300.00
12. Total Disability, maximum 104 weeks - monthly indemnity: 200.00
13. Partial Disability, maximum 30 days - maximum payable: 100.00
14. Hospital Confinement, maximum 30 days - maximum payable: 1,200.00
15. Medical Expense Reimbursement, non-disabling injury - actual up to: 100.00
16. Hernia Benefit, accident only - death or disability: 75.00

LIMITATIONS OF BENEFITS

50 percent of Death Benefits if death occurs from accident while member is intoxicated.   Benefits for Class "A" members are reduced for loss sustained while engaged in non-preferred occupation according to amounts shown in Classification of Risks.   Class "B" member benefits are not payable for loss sustained while engaged in non-preferred occupation according to amounts shown in Classification of Risks.   Class "C" and "D" members receive no accidental injury benefits.

EXCEPTIONS:

War; Insurrection; Invasion; Acting as Soldier or Sailor; Suicide; Felonious Act; Illegal Occupation.  Acting as Aviator, Balloonist or Parachutist or Participating in Aeronautics as Operator or Assistant or passenger or Rider in or on a Vehicle or Device for Aerial navigation, except 1, 2, and 3 above.  Infections that are not the result of accidental injury.  Disease of Body or Mind; Paralysis; Vertigo; Cerebral, Spinal or Meningeal Hemorrhage.  When resulting from Medical, Mechanical, Dental or Surgical Treatment.
